1. Place all documents inside a long-size brown envelope, labelled with FULL NAME and COURSE and submit them to the Campus Clinic / Registrar’s Office.
2. Enrollment applications for incoming freshmen and transferees will NOT BE APPROVED without the submission of all required documents.
3. Continuing Students with INCOMPLETE GRADES will NOT BE APPROVED to enroll.
